Before You Begin
You must submit a Payment Election by January 21, 2027 to receive a Class Payment. Choose one method: PayPal, Venmo, or Zelle; ACH transfer; or physical check. See Payment Options for the information each method requires.
Submitting this form does not by itself establish that you are eligible for a payment. Payments are made only if the Court grants final approval of the settlement.

Payment Election
Enter the Notice ID and PIN printed on the notice you received by email or postcard.
Your election must be submitted by January 21, 2027.

Your Payment Options
PayPal, Venmo, or Zelle
You will need the email address and telephone number connected to your account.
ACH Transfer
You will need your routing number, account number, and account holder name.
Physical Check
You will need your street address, city, state, and ZIP code.
Payment Election Questions
What is the deadline?
Submit your Payment Election by January 21, 2027. If you do not provide the necessary information by the deadline, you will not receive a Class Payment.
What if I don’t submit a Payment Election?
You will not receive a Class Payment. Unless you opt out, you will still be bound by the settlement and give up the Released Claims.
Is my information kept confidential?
The Settlement Administrator uses Class Member information only to administer the settlement. Bank account details are not shown in full on confirmation screens or in emails.
